Initial Visit/Psychiatric Evaluation (90 - 120 min) $400
Your first visit will include gathering a thorough medical and mental health history and determining treatment goals. May or may not include prescribing of medication.

1st Medication Follow-Up (45 min) $225
I require our first follow-up be 45 minutes to touch base on any new medication's tolerability and effect, and to continue information gathering + treatment planning.

Follow-Up Medication Management (30 min) $160
If we're continuing the same medications & treatment plan; includes brief/supportive psychotherapy

Intake for Troches for KAP (30 min) $200
I work closely with select local/Kentucky-licensed psychotherapists who have pursued high-quality, specialized training in ketamine-assisted psychotherapy (KAP). I complete a psychiatric and medical screening of these therapists' clients and prescribe ketamine as/when appropriate for monitored use during psychotherapy. NOTE: I only offer this service for clients who are directly referred by their therapist. A referral by a therapist is not a guarantee of a ketamine prescription.**

Medication Management for ADHD or KAP Meds ONLY $130
Subsequent follow-up visit on tolerability, efficacy, dose adjustment EXCLUSIVELY for ADHD stimulants and troches for KAP.

Time Outside of Scheduled Appointments $20 per 5 min.
My prorated hourly rate is $20 per 5 minute increment for the following: emails & Spruce messages requiring >5 minutes of provider's time; collaboration with outside providers; records review; letter writing; completion of forms (FMLA, short term/long term disability, etc). NOTE: I do not make changes to medications outside of scheduled appointments.
Life happens. I request 48 hours’ (two full business days) notice for cancellations. (For example, Monday appointments must be cancelled by the previous Thursday.) Cancellations made prior to this window are rescheduled with no penalty. The full charge will be applied to the card on file for no-shows or cancellations made with fewer than 48 business hours from the start time of your scheduled session.

** I am committed to upholding a high standard of ketamine-related care. Please check with your therapist to make sure they meet my licensure and training requirements (here) for ketamine-assisted psychotherapy (KAP). Without exception, your therapist must be available for collaboration during your ongoing care. My fee for ‘Time Outside of Appointments’ (outlined above) will apply.
