Discovery Call (15 min) $25
This is an *optional* 15 minute call if you would like to briefly address any questions you might have after a full review of my website.

Initial Visit/Psychiatric Evaluation (90 min) $300
Your first visit will include gathering a thorough medical and mental health history and determining treatment goals. May or may not include prescribing of medication.

1st Medication Follow-Up (45 min) $240
Your first follow-up visit is 45 minutes. This is to continue information gathering + treatment planning, as well as your response to any newly prescribed medications.

Follow-Up Medication Management (30 min) $160
This visit is for established clients who are continuing on the same medications & treatment plan. The visit includes brief/supportive psychotherapy.

Extended Follow-Up Medication Management (45 min) $225
This visit is for established clients who wish to discuss possible in medications and treatment plans or established clients who wish to have extended time for discussion and/or therapy.

Psychotherapy/Preparation/Integration (60 min) $230
Psychotherapy or KAP Preparation/Integration. This visit does not include prescribing of or changes to medications.

Intake for Troches for KAP (30 min) $200
NOTE: This service is exclusively for clients who are directly referred by their therapist. A referral by a therapist is not a guarantee of a ketamine prescription.** I work closely with select Kentucky-licensed therapists who have pursued high-quality, specialized training in ketamine-assisted psychotherapy (KAP). I complete a psychiatric and medical screening of these clients & prescribe ketamine if appropriate for monitored use during psychotherapy.

Follow-up for ADHD or KAP Meds ONLY (20 min) $130
EXCLUSIVELY for follow-up on ADHD stimulants and KAP medications. These visits are to determine tolerability, efficacy, possible dose adjustments.

Ketamine Infusion (120 min total clinic time) $400/ea.

Time Outside of Scheduled Appointments $20 per 5 min.
When >5 minutes of time is spent on the following: emails, Spruce messages, collaboration with outside providers; records review; letter writing; completion of forms (FMLA, short term/long term disability, etc), or prior authorization appeals, a fee of $20 per 5 minute increments will be billed. NOTE: Unless specifically discussed during a prior visit, I do not make changes to medications without an appointment.
I require 48 hours notice (two full business days) for cancellations. (Monday appointments must be cancelled by the previous Thursday.) Cancellations made prior to this window are rescheduled with no penalty. The full charge will be applied to the card on file for no-shows or cancellations made with fewer than 48 business hours from the start time of your scheduled session. (Life happens: short notice cancellations will be considered on a case-by-case basis at the discretion of the provider. There is no exception for no-shows.)

** I am committed to upholding a high standard of ketamine-related care. Please check with your therapist to make sure they meet my licensure and training requirements (here) for ketamine-assisted psychotherapy (KAP). Without exception, your therapist must be available for collaboration during your ongoing care. My fee for ‘Time Outside of Appointments’ (outlined above) will apply.
