If offered by your insurance plan, we will provide you with a superbill to submit to your plan for partial reimbursement or to apply to out-of-network deductibles. It is your responsibility to investigate this option with your insurance provider.

Per Kentucky law (907 KAR 3:005), Medicaid recipients found to be paying cash for services that would be covered with Medicaid-enrolled providers (ex: medication management or psychotherapy) can be subject to having their case audited. Such an audit may identify seeking care outside of Medicaid as fraud, RESULTING IN THEIR MEDICAID OR OTHER ASSISTANCE (such as SNAP) BEING SUSPENDED OR TERMINATED. The Medicaid enrollee would then be required to pay the government back for the cost of any Medicaid and SNAP benefits they have received.

Communication 

 
 I’m a solo practitioner with one amazing medical assistant (Alexis – she makes everyone’s life easier).  Voicemails will be responded to within 24 business hours, but answering the phone during business hours takes one or the other of us away from direct patient care. She and I are very attentive to both emails and messages in Spruce – these are the quickest and best ways to get in touch with us.
 
I use Spruce for messaging with established clients. The platform is HIPAA-compliant, protected by end-to-end encryption. I recommend downloading the app, but doing so is not required.

Osmind is my EHR/charting system. It will send you an intake email and occasional mood surveys, but I do not use the messaging feature (Spruce is a much more user-friendly experience for both of us). You do not have to download the Osmind app.

For prescription refills, please contact your pharmacy – they will submit a refill request on your behalf.

  • Refill requests from your pharmacy will be responded to within 24 business hours. 
  • Refill requests made after business hours and on weekends will be responded to the next business day.

Getting Started


Once you’d completed the interested form above and have decided to move forward with an intake visit, I will create your portal accounts.

You’ll get a text message that looks like this:

You do not have to download the Osmind app, but you do need to create an account using the link sent to you. You will also need to fill out the Osmind Intake form and questionnaires.

You’ll get an invitation to Spruce. This program has mobile and desktop options and is the quickest, most secure way to communicate with my practice. 

Once you have completed all your intake forms and uploaded or provided a payment method (which is stored with Stripe, not with me), we’ll reach out to get your intake visit scheduled.

I do not confirm or reserve intake appointments until all new patient forms have been completed.
